End of insurance cards

ZUS (National Insurance Institution) will not issue and confirm insurance cards from the beginning of the new year. From 1 January 2010 entitlement to medical benefits will be confirmed by other documents defined by National Health Fund, in particular, documents confirming payment of contributions.

One of the documents confirming the right to medical care will be ZUS RMUA form, i.e. the information about amount of contributions paid monthly by the payer. In case of people engaged in business activity such confirmation will be application form to health insurance and contribution payment note.
